7. Monitoring and evaluation report before adjusting investment projects – Form No. 15 issued with Circular 05/2023/TT-BKHDT
8. Original Investment Registration Certificate
9. Notarized copy of Business Registration Certificate
10. Notarized Copy of ID card of the person authorized to submit docs
11. Printout of Reg Number from FDI Web
